I received my Covid-19 Booster shot on December 22, 2021, in NYC location Essex Crossing (which is considered through NYC). NY mayor DeBlasio approved Booster shot incentives (Debit card) from Dec 21 - 31,2021. I contacted by phone Akimbo customer service on December 28, 2021 and requested a physical card be mailed to my home address. (I couldn't understand how to use the "virtual debit card" so that's why I requested a physical card mailed to me. I still have not received the card in my mailbox. Resolution Sought I want the physical $100.00 debit card mailed to me at my home.

My husband and I went to get vaccinated my and you receive to incentivize of 100.00 per person. We were told the would be available in the morning but in the morning the finds were still not there. We couldn't go online it kept giving error messages and when I got someone on the phone they now are saying 5 days to receive the money now?? My kids had gotten Covid 19 my husband missed a lot of work, we counting on that money to survive. Now they tell me basically there is nothing I or anyone can do? This has been a total nightmare and no one wants to help us. I've never herd of an incentive prepaid card with a deposit in August then a reversal in September? Resolution Sought What I would want to happen is to receive the money that we are owed that's it! I need to take care of our family and not taking the fact that I expected what they told me to be true. Not be totally stuck because of some reason or another.
