Hello, I am been going back and forth with ATI Physical Customer service people trying to get them to file a claim with their insurance company for over 7 months now . Here is there full address . I went to one of their locations in the state of WA . Here is their address if they want it ATI at1570 W Armory Way Ste 103, Seattle, WA 98119 . The date of service was on 11-10-24. I went to their Seattle WA Location to get my back checked out and saw Catalina Nazal for an evaluation for only back pain from a LYFT driver speeding where my back hit the back seat . I wanted to get my back better for the pain I had so I could be ready for Track and Field but instead the Physical Therapist ended up doing more damaged then good and she ended up doing chiropracting without my permission. I ended up going to the Urgent care clinic the next day and they told me to go to the Emergency room because they could tell that my back was damage for what the physical Therapy did to me. I was diagnosed with "right eccentric disk protrusion at T7-T8 resulting in mild-moderate spinal canal narrowing, and central disk protrusion at T8-T9 resulting in mild-moderate spinal canal narrowing." I just need you guys to stop playing games and file a claim with your insurance company for what has happened to me Their is a Police investigation go in on By Seattle Police I am trying to find a compromised resolution before they find your company guilty. Resolution Sought I would like the business to file a claim with their insurance company or five me the name of their insurance company.
