Mrs. Rauch was seen by Dr. William Atkinson (optomertrist) for an in-home visit on August 9, 2022. We have exam notes from that date, we also received her insurance cards and medication list either from the patient herself or her POA c(contact) Bob, her son. In most cases if a patient did not have an appointment we would not have this information in our system. The patients son, Bob, did also call us because the patient was having trouble seeing her tv out of her new glasses. She ordered and received glasses from us on her exam date and we dispensed them toher on 9/8/2022. We have also tried reaching out to get updated insurance information to bill for her visit, but unfortunately were unable to get payment from the insurance; therefore, we had to bill the patient. Unfortunately, Medicare gives us a year from the date of service. This was a service provided by our practice.

To the best of my recollection a phone call was made requesting an appointment , but the request was denied. That was three years ago and there was no other contact until a collection bill arrived. No information was given, not Medicare nor a med list, there was never a “ reach out for information”. There is no son named Bob.. There were no bills sent or Medicare claims submitted. That being said, I propose the following: If the information in your file regarding medical information or the medication list is accurate then I will acknowledge that some type of appointment occurred, even though I have no memory of it. If the information is not accurate then all billing issues will be resolved
