I am a 66 year old man that is bed ridden, for 2 weeks now essen house calls has not filled out a DOH form for my CDPAP home care services to be sent to healthfirst so they can reinstate my services. I have not be serviced for 2 weeks now, I live alone have no family. Please help me. Resolution Sought Comply with my medical needs and services due me.
