My husband had a 401K with Fidelity thru his work at Shoppers Food and Warehouse. He passed away in February of this year. I received a statement on April 18, 2022. I contacted Shoppers on April 19, 2022 to find out who was handling this account. I was transferred to Sandra at Fidelity Investments. I was informed that it would take 10 days to review the account and see who was listed as beneficiaries, Sandra gave me a work order # W76462919APR 22. On the 19th I also received an email from Fidelity requesting a copy of my husbands death certificate. I sent that using the link that they had provided. I called back on April 29th to get an updated and spoke to Channel. She indicated that a Survivors package has bee sent to me on April 25th with some documents that I would need to fill out and return . On May 6th I had not received the package and spoke to Crystal who advised I should have it by middle of that week. On May 16th I called back speaking to Clifton. He advised that a new package would be going out that next day expedited mail and I should have it on Thursday May 19th. He verified my email, phone and address. I still have not received any package from them to finalize this account and have been given numerous excuses why this has not happened. Jay stated that it had to start over from the beginning and gave me a new work order # W16750416 MAY 22 Resolution Sought I want to get the documents needed to finalize this account and have the funds distributed to me as his beneficiary. I am tired of the run around and lies that I have been told

I recently worked for a company (Domingo Logistics) and was supposed to get paid monthly via direct deposit. However, it turned out to be a scam because I never got paid. Recently, I started receiving mail from Fidelity Investments about my 401(k). I decided to contact them to find out what company signed me up for this investment. They wouldn't give me any info. and this is in my name, instead, I was asked to give them Company names and dates of employment. I am 62 years old and have had plenty of jobs. I'm inquiring because this is something I should know about especially since I was scammed with an employer before. I want to know what specific employer I had connected to this 401(k) plan. Resolution Sought Find out who signed me up for this 401(k) plan and when.
