I went online and started quote for state farm insurance, an agent called me back named Stephanie white out of the Oregon Illinois office. I had State Farm insurance before and never questioned their professionalism so when the State Farm agent Stephanie white said she would be emailing me the policy to sign I did just that when received. It wasn't until about a month later when I received my first statement in the mail that I realized something was not right. There was my personal information with an insurance company named that as of this time I never heard of which was Gainsco I immediately called the Oregon office where I spoke to the branch manager Dexter. I informed him that Stephanie white not only gave me this insurance with out my knowledge but also with out my consent. I am unaware if Mrs. White was reprimanded. But the following three months my insurance policy was terminated due to signatures that where not requested at time of completing the policy with Mrs. White. After speaking again with branch manager Dexter he stated that the Gainsco policies where new and if I would be patient as they work out the kinks. I also informed him that I have not received any documents like policy or identification cards to put in my vehicle. I was then emailed my identification card. On dec 07 I needed the coverage of this policy and found out that my coverage had been changed without my knowledge or consent to a severely less coverage these changes where made before Mrs. Le Resolution Sought Continuing: ft the company she no longer works there. I would like a full investigation and to be reimbursement in full for service not provided. And also compensation for the hardships me and my family had to suffer through this ordeal.

February 21, 2024 Business Consumer Alliance 500 James Robertson Parkway, 10th Floor Nashville, TN 37243-0574 Consumer: Mr. White Document ID: BCA-99015704 Policy #: ILPA03353197-003 Insured: Mr. White To Whom it May Concern: This shall serve as the response of MGA Insurance Company, Inc. ("GAINSCO"), to the Consumer request for assistance dated January 30, 2024. Mr. White purchased a policy with GAINSCO through Stephens Insurance & Financial Services Inc. agency, with an effective date of June 1, 2022. The policy was originally bound with two vehicles, with BI/PD Liability, Uninsured Motorist, Medical Payments, and Rental Reimbursement coverage for each vehicle. At no point were Collision or Comprehensive coverages included on the policy, or the premiums paid by Mr. White. On July 21, 2023, a change was made to the policy through Mr. White's agent to remove the 1997 Chevrolet Suburban from the policy. The Rental Reimbursement coverage limit for the 2004 Mercedes C Class was also changed from 40/1200 to 30/900 reimbursement limits. On July 26, 2023, the agent contacted GAINSCO to confirm the changes had been made on July 21, 2023. An amended Declarations Page was generated and mailed to Mr. White, indicating the changes to the policy. The policy was renewed on December 1, 2023, and the renewal Declarations Page matched the policy changes made in July 2023. On January 4, 2024, Mr. White called GAINSCO Customer Service to discuss the changes to his policy. The GAINSCO representative advised Mr. White that his policy documents were not signed, but when a change is made to a policy, the electronic documents are not required to be uploaded to the policy to complete the change. GAINSCO does suggest for the agent to send the email confirmation amendment to the insured for signature. We reached out to the Stephens Insurance agency, and they advised the agent who handled the policy change on Mr. White's policy no longer works at the agency. They were unable to provide any additional information regarding the July 2023 policy changes. White is currently paying premiums for the coverages found on his renewal Declarations Page. If Mr. White wishes to add coverage back to his policy, he should contact his agent to do so. Worst company ever! Had a car accident two weeks ago! Have not heard from adjuster at all. Never reached out. I always have to call them to find out what's going on with my claim! They are giving me the run around about the check for the auto body shop where my car has been for the past week. Resolution Sought Give me a rental since they are dragging their feet

GAINSCO sincerely apologies for any inconvenience caused to Ms. Singleton. GAINSCO was advised of the animal hit / loss on Aug 1, 2023, by Ms. Singleton. An inspection of the vehicle was scheduled for Aug 3, 2023. An estimate was completed in the amount of $5,147.37. On Aug 8, 2023, GAINSCO was in contact with Ms. Singleton who confirmed she would like to use Rudy and Sons Collision Center. GAINSCO also advised Ms. Singleton of the repair and supplement process. GAINSCO issued a draft on Aug 8, 2023, in the amount of $4,647.37, the estimate amount less Ms. Singleton’s deductible. On Aug 21, 2023, GAINSCO spoke to insured who advised the check had not arrived. GAINSCO advised Ms. Singleton of the “stop pay” process if she did not receive the check and that GAINSCO could re-issue. On Aug 22, 2023, Ms. Singleton confirmed that she had received the check #0220295, as it was in her mailbox. GAINSCO advised of the supplement process and is ready to assist Ms. Singleton as needed.
