CRI is just a subset of the overall rating for the company. I called Habana Health Care facility last week Feb. 15@9:57am, 9:59am, Feb. 16@2:54pm, 2:56pm, 4:39pm and today. Feb 19@10:01am. To speak to Michelle Flowers, the executive in charge. Her voicemail cuts off so that you cannot leave a message. Each time the receptionist (Teresa) put me through to her voicemail. I told her the system does not allow me to leave a voicemail. What type of facility are they running there? I'm sure I am not the only person having a problem with this system. This is in regards to one if their residents Gregory Stafford. He has asked me to make sure my name and phone number is on all of his paperwork there in case he is hospitalized or something happens where he cannot speak for himself. This is very important and not to be taken lightly. Also he has asked to see a doctor and that has not happened. He wants to be able to know if he can live on his own with the help of a worker but cannot get a doctors appointment. The last appointment in he had was cancelled and has not be reset. Resolution Sought I would like to make sure my name is on all of his paperwork to contact me if anything happens and he has to see a doctor or if his health declines. He can and will attest to that because we just talked about it last week. I would like my name to be put on his financial accounts as well. If he needs to sign over anything he is willing to do that, no one from that facility has come forward for him to take care of this request.
