Order no. 164 519 556. I ordered divorce papers Wed, 06/28/23 and 2 weeks later I received package Wed, 07/12/23. I had to sign for it from UPS Express. I thought I was getting the detail pages but received this one page certificate. I filled in initial requested information correctly. I received divorce certificate and it was not correct at all. It needs to be shredded. My name was down as husband. Husband name was Bruce and should have been Don. My name was down as being the attorney which is incorrect. Tons of misspellings. Sections filled in unknown, unknown, unknown. Also, it was confirmed I didn't need this after all. Unfortunately there is 1 default star this deserves zero stars. Resolution Sought Refund me full amount of 79.75 or subtract shipping fee 19.75 and make it a refund of 60.00.

I ordered a death certificate, and the vital statistics of Yonkers New York sent a ripped up form that they could not locate it. Notified Vitalchek to request a refund because they would not respond to me and VitalChek Notified me they were not responding to them either. I was told the first time that if they did not respond in 5 days I would be refunded. It has now been 3 weeks and thy say they sent some form 4 times which is a lie. The only form I received was the torn up form. Now they tell me the Money I paid was not refundable. I paid for a death certificate that was never received and this form is not what I paid for. Resolution Sought Refund my money.
