12/21 called ohio eye request exam asked if exam was covered under my insurance they said yes. Went in for apt 12/28/21 . Received a 30.00 bill 3/8/22 for a refraction a test given outside of the exam without my knowledge or consent. Resolution Sought Remove 30.00 charge and send verification of removal.

As part of the eye exam a refraction was performed and billed appropriately for $30.00. The charge was submitted to both primary and secondary insurances. Both insurances state the charge is the patient's responsibility. However, as a goodwill gesture Ohio Eye Alliance will remove the charge.
