Had tests done @ Nazareth hospital on 4/10/21 - 4/20/21 - 8/5/21 Last year mailed insurance information & also called same to their phone # I have Medicare & united Healthcare plan f which covers whatever Medicare doesn't. They have not billed Medicare so united Healthcare will not pay Nazareth hospital has received their payment by my insurance RAI customer service phone # had to be called 551-298-3605 because website does not allow you to update insurance info They just want payment on website Had more tests on 4/25/22 & want to avoid getting billed inappropriately Account #176095 Resolution Sought Bill Medicare so payment is received and/or passed to united Healthcare which will cover 100
