In 2018-19 I lived there with my ex-fiance. I went through the proper channels about my service dog. My pysch. Doctor filled it the paperwork and faxed it several times plus I brought a copy in and they are charging me in collections for my dog. I moved due to domestic violence. I should not be charged for my dog since I had the proper paperwork. They stated they didn't receive it when I was standing at the doctor's office at least 2 times when they faxed and again when I called them and physically brought a physically the copy in. I discussed it with the company before I got my dog. Resolution Sought To have it remove from my credit.

We can only accept medical documents directly from the provider. Ms. Harrison's statement that she physically brought in the signed documents has not been verified by the notes from the community. If she were to have brought in documents they would not have been accepted as they were not sent in by the provider. We show that Ms. Harrison did not attempt at any time after April 18th, to re-open the request as was stated that she could in the cancellation letter. As for the claim that she had to vacate due to domestic violence; we do not show where at any time legal documents were provided to either Spring Lake Cove or to our internal Collections Department to validate the reason for the move out (i.e. police reports, copy of restraining order, ect.). We also do not show where a request was submitted to Spring Lake Cove by Ms. Harrison asking to be released from the lease contract. The charges for the unauthorized pet and for early termination of the lease are valid. We show this account is currently with a third party collection agency, National Credit Systems. All further correspondence in regards to this account do need to be directed to them. They can be reached at 1.800.367.1050. Collations Supervisor
