Before my mom went into the hospital she had a super small bedsore and we were taking care of it and it was going away. My mom went to Kaiser ER in Ontario, ca on November 11th 2025 and transferred to ST. Bernadine Medical Center in San Bernardino,ca. where she stayed about a month and she was doing better.. Then she was sent to Highland palms in San Bernardino for short term Rehabilitation which she was transferred for about 9 days and she was in bad condition when she was transferred back to St. Bernadine medical center where care was neglected. She had been moaning a lot and they wasn't giving her pain meds. I would request them and it made her feel better also when they so called went in her room to change her dressing and turn her they would ask us to leave. I told one of the nurses I would like to see her sore but they told me o had to leave. ..She was put on hospice on Friday December 26th. She got here on Dec 27th and the hospice nurse was here and changed her dressing for her sore and it was so big and I was sooo surprised at it and got very upset. I asked the nurse which stage do u think her ulcer is? She said it's definitely a stage 4. Took pics of the sore and my mom passed on December 29th. She should still be here. She was walking and talking when she went in. Her doctors are upset and surprised she is gone as well. Resolution Sought Nursing care needs to better.Her bed sore was not being taking care of .Being in the hospital that short time shouldn't have went to a stage 4 in 2 months . Compensation would help

I want to speak to someone in charge of the hospital to show them 4 different mistakes they did to my wife that they could train their nurses not to do again. I wrote a grievance letter to tell them I need to speak to someone and they wrote back they can't address me with my concerns. You guys didn't care about my wife's life you can at least try to save others cause one of these days it's gonna be one of your family members and your not gonna like it when they push you to the side and lie. Thats what you guys are there for to save people's lives not take them. Going n to check on a blister to dying a fews hours later being brain dead somethings really not right Resolution Sought Speak to me so I could get this off my mind. And put it in the paper what really happened I got it on paper let me show you and point it out. I left messages to everyone I could but no response. Why do you leave numbers to call if you have a problem if you don't answer?

Company states; We are in receipt of the attached Grievance, dated March 27, 2024. We received this grievance on April 1, 2024. Again, please accept our condolences on the loss of your wife. While we appreciate the grief you must be experiencing, there is nothing more the hospital can do to address your concerns. My attorneys the one who told me not to speak with the nursing board he said he will take care of it. Then he requested to have my case dismissed and he did it with prejudice without my permission. The attorney I got I didn't contact him he left two messages on my phone saying he wanted to take my case. I found out he was a former hospital attorney he didn't listen to nothing I told him he waited to statue of limitations was up then he dismissed it. I tried going to court n they told me I had to find someone to represent me but no attorney would try cause my attorney dismissed it with prejudice. I been trying to speak with anyone that would listen but you keep pushing me aside.why can't u spend 30 minutes of your time so I could show you on medical records you guys have me what they did wrong so they don't do it again. A nurse shouldn't throw the IV bag while hooked to patient and allow air to get in line and make patient have a heart attack or stroke. Another once nurse found out she's not breathing call code blue don't leave them in hallway and hook her up to a ventilator 18 minutes after. Now she brain dead. You said it was alergic reaction from antibiotic u gave it 10 more
