To check on the facility to prevent anyone else enduring such bad care.

I was a resident at the drug and alcohol inpatient facility Stepping Stones Recovery Home located 17277 East Cypress St Covina , Ca 91277. My medi-cal insurance covered and i want to file a grievance due to bullying (from one resident towards me) , discrimination/favortism , negligence resulting in me having occurred legal issues that i didn't come in with, improper health and food administering to us, never got a covid test in duration of time there nor did anyone else, residents having hair & insect in meals. (two different incidents) . No proper training at all to residents who prepared and cooked our meals not requiring them to wear hair nets no guidelines of how to safely cook like proper hand washing or cross contamination information. A slue of other issues . The day i was admitted to their facility i signed a release of consent form for them to notify my probation officer that was on 4/19/21 (intake date) this day i was put on a 14 day non-communication blackout period (no phone calls to anyone) . I informed and urged them to contact my Probation officer because i could get a violation if she couldn't contact me or know my whereabouts. got off my black out period (5/1/21 a day early) and found out they never notified my p.o. of me being there and she sent out a violation of probation 4/30/21 and i received a court date 5/19/21, i could get 4 years in prison due to my joint suspension if found guilty of violating my terms of probation. I was 3 months from getting
